New box options:

• Position: set initial box position (normal, advanced, side).
• Include: a comma separated list of post IDs that will be included even if their post types are not supported by the box.
• Exclude: comma separated list of post IDs, exclude the posts even if their post types are supported.

The Fields menu has become a top level menu, more sub menus will be added later.
